The above link is for the online Risk Management form that has to be filled out by everyone involved with your soccer program, including ALL coaches, assistant coaches, team managers, referees, board members, employees, any volunteer who is expected to have regular contact with youth players.
Rosters will not be allowed to be printed for teams if the coaches and assistant coaches have not filled out this form online. There should not be any forms filled out in any manner other than online. If someone doesn't have access to their home computer, they will need to do this at the library or wherever they can find access to a computer.
